Description

This template is organized into the classic four-phase PDCA format:

  • PLAN: Define the problem, its root cause, and propose countermeasures. Includes structured fields for identifying the problem area, responsible users, root cause analysis, immediate containment actions, and multiple countermeasure options.
  • DO: List and assign specific actions to be taken, with associated deadlines and status dropdowns (e.g., “To do,” “Doing,” “Done”). Each task includes fields for the owner and expected completion date.
  • CHECK: Verify whether the root cause has been solved. Includes status evaluation, file uploads for supporting evidence, and review notes.
  • ACT: Determine next steps based on outcomes—such as standardizing procedures, expanding improvements to other areas, or repeating the PDCA cycle.
